How we write our NCLEX-style practice questions
Every practice question on NursingCEA is written from scratch, explained in full, and traced back to a real clinical source before it ever reaches the question bank. Here is exactly how that works.
NursingCEA does not provide CE credit. This page describes our editorial process for free practice and study material — not continuing-education credit, contact hours, or certificates. Every question is original
We never copy or paraphrase questions from UWorld, Kaplan, ATI, NCSBN, or any other commercial question bank — doing so would infringe their copyright, and it would also mean students see the same items in more than one place instead of genuinely new practice. Every stem, every answer choice, and every rationale on NursingCEA is written for this site.
A drafted question is only a starting point — nothing reaches the question bank until it has cleared the sourcing and review steps below.
Every question carries a full rationale
A right answer without an explanation only tells you what to pick next time — not why. Every question on NursingCEA includes a rationale that explains why the correct answer is correct and why each of the other choices is wrong, so a missed question still teaches the underlying principle instead of just the answer key.
Every question is cited to a real source
Every rationale names the textbook chapter or clinical guideline it was written from. These source families are the backbone of every question we publish:
Core nursing textbooks

Write, then independently review
No question goes live off a single pass. Every question moves through the same two-stage editorial process before it's added to the bank:
1. Draft
A question is drafted around one clinical concept pulled from a specific textbook chapter or guideline section — never copied from a commercial question bank. The draft includes the stem, every answer choice, and a first-pass rationale with its source citation attached from the start, not added afterward.
2. Independent review
Before it publishes, every drafted question is reviewed separately from the person (or pass) that wrote it — checking the clinical accuracy of the correct answer, the accuracy of the stated reason each distractor is wrong, and that the citation actually supports what the rationale claims. Anything that doesn't hold up is revised or rejected, not published as-is.
3. Publish
Only questions that pass review are added to the live bank. The same standard — original stem, full two-sided rationale, real citation, independent review — applies whether a question feeds a topic quiz, a full-length practice exam, or one of the sample questions shown on this site.
Frequently asked questions
- Are NursingCEA's practice questions copied from UWorld, Kaplan, ATI, or the NCLEX itself?
- No. Every question is written for this site. We never copy or paraphrase questions from UWorld, Kaplan, ATI, NCSBN, or any other commercial question bank. Sources are limited to nursing textbooks and clinical practice guidelines, cited by name.
- Does every question include a rationale?
- Yes. Every question — regardless of topic or format — includes a full rationale explaining why the correct answer is right and why each of the other choices is wrong, plus a citation to the textbook chapter or guideline it was written from.
- Are the questions reviewed before they're published?
- Yes. Each question goes through a two-pass process: it's drafted with its rationale and citation attached, then independently reviewed for clinical accuracy before it's added to the live bank.
- Does NursingCEA provide CE credit or a certificate for completing questions?
- No. NursingCEA is a free practice and study resource, not a continuing-education provider. We do not offer CE credit, contact hours, or certificates for any quiz, practice exam, or study guide on this site.
